It is your responsibility and very important to let the practice know if you change any of your contact details, to avoid breaches in confidentiality and to also ensure any referrals or letters from the practice are sent to the correct address.
Please inform us ASAP of any changes to your name, address, telephone number or email address. you can do this by emailing us and letting us know: surgery.wheatbridge@nhs.net, via emis access, posting a letter in or dropping a letter in at reception. However if you move out of our practice area you may be asked to register closer to your new address.
